CERTIFIED HOME HEALTH SERVICES
Certified home health services are provided for treatment and recovery after an illness or injury. Medicare will pay for home health services provided by a Medicare-certified home health agency if the following conditions apply:
1) The patient needs intermittent skilled nursing, physical therapy, speech therapy or continuing occupational therapy (when a prior need for nursing, physical therapy or speech therapy has been established).
2) The patient is homebound.
3) The patient is under the care of a physician who determines the need for care, subsequently sets up and periodically reviews the plan of care.

OUTPATIENT DIABETES EDUCATION CENTERS In our Houma, Louisiana and San Antonio, Texas locations, TMT offers outpatient centers for Diabetes Self-Management Education (DSME) and Medical Nutrition Therapy (MNT). Patients eligible for DSME include individuals diagnosed with Type 1 and Type 2 diabetes. MNT can be provided for those with Type 1 and Type 2 diabetes, gestational diabetes, non-dialysis kidney disease and post-kidney transplants. The curriculum for these programs is based on The National Standards for DSME and the American Diabetes Association. The centers are staffed by multidisciplinary teams of certified diabetes educators and registered dietitians, with input from consulting physicians, pharmacists and podiatrists.
Typically, the MNT patient will receive an initial evaluation and four half-hour group training sessions. The DSME patient will receive an initial evaluation and nine hours of group or individual training. Through a program of individualized assessment, goal setting and training, our patients achieve:

PRIVATE DUTY AND PERSONAL CARE SERVICES
Our private duty services run the gamut from round-the-clock care to a weekly visit ..... from high-tech RNs to patient sitters .... from hourly care to our “custom care” task-based services. Specially trained clinical personnel are available for a wide range of special needs: working with Alzheimer’s sufferers; caring for stroke, cardiac, spinal cord and closed-head injury patients; providing convalescent care as needed. All services are provided in accordance with state licensing regulations and meet the highest compliance standards.
Our professional staff works with each client to develop a plan of services tailored to specific needs and budgets. A nurse assessment and written plan of care are provided at no cost.
PRIVATE DUTY SERVICES

24-hour a day care is available by RN's, LPN's/LVN's, or Aides, as appropriate to the case. For example, we provide 24-hour a day nursing care for patients on ventilators, where skilled nursing is required.

Skilled nursing care, all therapies, and the assistance of home health aides are available as appropriate for individuals after hospitalization or surgery, individuals with serious or multiple health problems, those who are disabled and/or those who need assistance to remain at home.

Assistance with personal care and activities of daily living is provided to individuals in their homes, whether a private residence, independent or assisted living facility or nursing home. Services typically include any or all of the following: personal care (bathing and personal hygiene); assistance with dressing, toileting, eating, ambulation, and range-of-motion exercises;
medication reminders; meal preparation; laundry and light housekeeping; and, monitoring vital signs.

Homemakers and companions handle light household duties and/or companionship when no hands-on care is needed.

Patient sitters can be at the patient’s bedside in hospital or nursing home when the family wants continuous support for the patient.

PERSONAL CARE SERVICES
FOR RESIDENTIAL COMMUNITIES We provide custom care and personal care services to residents of independent and assisted living communities. These services include task-based care and the full range of private duty services.
COMMUNITY CARE AND IN-HOME SERVICES
The company contracts with public entities in each of our service areas to provide home and community support programs for low-income elderly and disabled clients.
Texas
MED TEAM, INC. (MTI) is a major provider of community support programs under contract with the Texas Department of Aging and Disability Services (TDADS). Currently we provide services from our offices in San Antonio, New Braunfels, Brownsville, and Corpus Christi.

Primary Home Care (PHC) is a personal care service available to clients with Medicaid or eligibility under Social Security, whose health problems cause them to be functionally limited in performing activities of daily living.

Community Based Alternatives (CBA) is a Medicaid program that provides a cost-effective alternative to placement in nursing facilities and includes: personal assistance; nursing visits; physical, speech, and occupational therapies; medical supplies and adaptive aids; and home modifications.

The Consolidated Waiver Project in Bexar County, which is based on the CBA model, includes services to medically dependent children and to blind/deaf, MHMR and disabled adults.

Louisiana
 |
TMT contracts with Area Agencies on Aging in several parishes in southeastern Louisiana to provide personal care, respite care and homemaker services to eligible low-income elderly clients.

Since 2000, TMT has been contracted
as the home care provider for Ryan White
Title II CARE Act Services for Orleans
Parish. Under this contract, TMT provides skilled nursing and therapy services as well as personal care to patients and respite to caregivers of patients with HIV/AIDS who meet financial eligibility guidelines.

The company contracts with Area Agencies of Aging in metropolitan Detroit to provide personal care and homemaker services to eligible low-income elderly clients.

Virginia

TMT provides personal care and homemaker services to eligible low-income clients through county-funded programs in Fairfax, Arlington and Loudoun Counties in Northern Virginia. Over the years, the agency has innovated care delivery models for “cluster care” – providing services to multiple residents within facilities and communities.
