Diabetes Self-Management and Education

Developed by staff Certified Diabetes Educators, this program is based on the latest standards of the American Diabetes Association. Teaching incorporates proven educational protocols, tailored to individual needs, and emphasizes the development of self-care management skills. THE MEDICAL TEAM has extensive experience implementing this program for our patients' benefit.

 Program Components

  • Health status and educational needs assessment
  • Development of an individualized care plan with specific educational goals
  • Skilled care as ordered by a physician and based on established protocols
  • Program features include: glucose monitoring, medications, nutrition and meal planning, and prevention of complications
  • Comprehensive and clear teaching materials (also available in Spanish)
  • Coordination and communication with the patient’s physician and other health care providers
  • Outcomes reporting on key indicators of diabetes self-management and patient satisfaction

 Program Staff

Certified Diabetes Educators oversee the program and provide training and teaching materials to our staff nurses, many of whom have extensive experience in diabetes care and patient education. Registered Dietitians provide instruction about diet and nutrition. Many of our agency nursing staff are bilingual.

 Outpatient Diabetes Education Centers

Offered at our Houma, LA and San Antonio, TX locations, our outpatient diabetes education centers help patients achieve self-management of their diabetes, improved health and nutritional status, and prevention and control of the complications of diabetes. The program includes individualized assessment, goal setting and training. Our outpatient centers have been granted recognition from the American Diabetes Association and our programs have met National Standards for Excellence in Diabetes Self-Management and Education.

Programs Offered:

Diabetes Self-Management Education (DSME): For individuals diagnosed with Type 1 and Type 2 diabetes. Patients will typically receive an initial evaluation and nine hours of group or individual training.

Medical Nutrition Therapy (MNT): For those with Type 1 and Type 2 diabetes, gestational diabetes, non-dialysis kidney disease, post-kidney transplants. MNT may also be available for persons with obesity wanting a structured and individualized nutritional weight loss program. Patients will typically receive an initial evaluation and four half-hour group or individual training sessions.

Curriculum: Based on The National Standards for DSME and the American Diabetes Association.

Staffing: Multidisciplinary teams of Certified Diabetes Educators and registered dietitians, with input from consulting physicians, pharmacists and podiatrists.
