24-hour a day or hourly care by specially trained Registered Nurses, Licensed Practical Nurses/Licensed Vocational Nurses, or Home Health Aides, as appropriate for individuals after hospitalization or surgery; individuals with serious or multiple health problems; those who are disabled and/or those who need assistance to remain at home. For example, a patient on ventilator might require 24-hour a day skilled nursing care.
Assistance with personal care and activities of daily living provided to individuals in their homes, whether a private residence, independent or assisted living facility or nursing home. Services can include any or all of the following: personal care (bathing and personal hygiene), assistance with dressing, toileting, eating, ambulation, and range-of-motion exercises, medication reminders, meal preparation, laundry and light housekeeping and monitoring vital signs.
Homemakers and companions handling light household duties and/or companionship when no hands-on care is needed.
Patient sitters who can be at the patient’s bedside in the hospital or nursing home when the family wants continuous support for the patient.
