Community Care Programs

THE MEDICAL TEAM contracts with state and local assistance programs in each of our service areas to provide home and community support programs for low-income elderly and disabled clients.

 Current Programs

Louisiana: THE MEDICAL TEAM contracts with Area Agencies on Aging in several parishes in southeastern Louisiana to provide personal care, respite care, homemaker and wellness services to eligible low-income elderly clients. Since 2000, the company has been contracted as the home care provider for Ryan White Title II CARE Act Services for Orleans Parish. Under this contract, we provide skilled nursing and therapy services as well as personal caretaker services to patients with HIV/AIDS who meet financial eligibility guidelines.

Michigan: The company contracts with Area Agencies of Aging in metropolitan Detroit to provide personal care and homemaker services to eligible low-income elderly clients.

Virginia: THE MEDICAL TEAM provides personal care and homemaker services to eligible low-income clients through county-funded programs in Fairfax and Loudoun Counties in Northern Virginia. We utilize innovative "cluster care" delivery models – providing services to multiple residents within facilities and communities.

Texas: Our affiliated company MED TEAM, INC. serves San Antonio, New Braunfels, and Brownsville. MED TEAM, INC. provides services through state funded community care programs: STARPLUS, Primary Home Care, Community Based Alternatives, and Consolidated Waiver Program. We provide service in the counties of Atascosa, Bandera, Bexar, Caldwell, Comal, Dewitt, Frio, Gonzalez, Guadalupe, Hays, Karnes, Kendall, Kerr, Medina and Wilson.

 Our Services

Personal Assistance Services

Patients: Eligible Medicaid and SSI recipients who have medically related health problems causing them to be functionally impaired.

Services Provided: We provide personal assistance with activities of daily living. For example, bathing, dressing, grooming, assistance with medications, light housekeeping, escort, meal preparation and ambulation.

Community Based Alternatives (CBA) and Consolidated Waiver Program (CWP) (Provided in Texas only)

Patients: Eligible Medicaid clients who would qualify for Medicaid nursing facility (NF) care. Through CBA/CWP, these patients are given a choice in long-term care.

Services Provided: Personal assistance services, skilled nursing, therapies, adaptive aids, medical supplies and minor home modifications.

Private Duty

Patients: Those requiring care at home but who are not covered under Medicare, Medicaid or other insurance. Payment is out of pocket.

Services Provided: Short-term and long-term services are available 7 days a week, 52 weeks a year for as little as 4 hours of personal care or as much as 24-hour skilled nursing care. Services and financial arrangements are defined in a written service agreement.
