Our outpatient diabetes education center helps patients achieve self-management of their diabetes, improved health and nutritional status, and prevention and control of the complications of diabetes. The program includes individualized assessment, goal setting and training. The center has been granted recognition from the American Diabetes Association and our programs have met National Standards for Excellence in Diabetes Self-Management and Education.
Programs Offered:
Diabetes Self-Management Education (DSME): For individuals diagnosed with Type 1 and Type 2 diabetes. Patients will typically receive an initial evaluation and nine hours of group or individual training.
Medical Nutrition Therapy (MNT): For those with Type 1 and Type 2 diabetes, gestational diabetes, non-dialysis kidney disease, post-kidney transplants. MNT may also be available for persons with obesity wanting a structured and individualized nutritional weight loss program. Patients will typically receive an initial evaluation and four half-hour group or individual training sessions.
Curriculum: Based on The National Standards for DSME and the American Diabetes Association.
Staffing: Multidisciplinary teams of Certified Diabetes Educators and registered dietitians, with input from consulting physicians, pharmacists and podiatrists.
